Cancellation Policy

Fox Valley Functional Medicine LLC requires that all patients respect our time-reserved appointment system. The following rules apply to all scheduled services:

  1. Notice Requirement: A minimum of 48 business hours notice (two full business days) is required to cancel or reschedule any appointment.

  2. Late Cancellation: Any cancellation or reschedule made with less than 48 business hours notice will result in a late cancellation fee of 50% of the scheduled service fee.

  3. No-Show: Failure to attend a scheduled appointment without prior notice will result in a no-show fee of 75% of the scheduled service fee).

  4. Billing of Fees: All cancellation and no-show fees are non-refundable, not billable to insurance, and will be charged to the payment method on file. 

  5. Scheduling Fee: $100 deposit is required at the time of scheduling to reserve the appointment. This can be refunded if appropriate communication is received (48 hour notice prior to appointment time).

  6. Appointment Punctuality: We value your time and aim to keep all appointments on schedule. There’s a 10-minute grace period for late arrivals. If you’re more than 10 minutes late, we may need to reschedule to ensure we can give you the full attention you deserve. Arriving on time helps us make the most of your visit.

  7. Supplements: Supplements and herbals are specifically tailored to meet individual patient needs and cannot be returned or resold. Therefore, all sales are final, and no refunds will be given.

  8. Repeated Violations: Patients with three (3) or more late cancellations and/or no-shows may be subject to dismissal from the practice.

  9. Exceptions: Exceptions will only be granted in cases of documented medical emergencies or circumstances approved solely at the discretion of Fox Valley Functional Medicine LLC.

Billing & Payment Authorization

Membership Care Plans may be cancelled at any time. 

To avoid charges for the following month, cancellations must be submitted by the 20th of the current month.

Cancellations recived after the 20th will result in the next months payment being processed automatically.

No partial refunds are provided for charges already processed.

Scope of Membership

Membership fees do not include labratory testing, supplements, or outside services, unless otherwise specified in writing.

The FVFM Care Plan and Health Coaching services are intended to support health and wellness and do not replace emergency or primary medical care.
